I lost a few hours to it. Luckily, I had a manual save before the Mortis fight. I tried to recreate the issue after getting back to the area, and it properly teleported me back to the Nexus the 2nd time, so it doesn't always happen. Pretty horrible bug if you don't have a backup save, though.
